An advance directive for healthcare, also known as living will, personal directive, advance directive, medical directive or advance decision, is a legal document in which a person specifies what actions should be taken for their health if they are no longer able to make decisions for themselves because of illness or incapacity. The durable power of attorney for health care is the person designated to carry out those instructions when a person is unable to speak for themselves.

LaShanna Williams of End of Life Washington and A Sacred Passing and PMA Executive Director Nora Menkin will walk you through the Advance Directive form and Disposition Authorization and answer your questions so you can complete them. If you're ready to make decisions, you may complete the documents during this workshop and have them notarized at the end.

All participants will receive copies of the End of Life Washington advance directive for health care and the durable power of attorney for health care documents. Registration is free but required.
