Complete Your Advance Directive

An advance directive for healthcare, also known as living will, personal directive, advance directive, medical directive or advance decision, is a legal document in which a person specifies what actions should be taken for their health if they are no longer able to make decisions for themselves because of illness or incapacity. The durable power of attorney for health care is the person designated to carry out those instructions when a person is unable to speak for themselves.

A representative from End of Life Washington will take us through each section of their advance directive for health care and the durable power of attorney for healthcare. If you're ready to make decisions, you may complete the documents during this workshop and have them notarized at the end.

All participants will receive copies of the End of Life Washington advance directive for health care and the durable power of attorney for health care documents. Registration is appreciated.

End of Life Washington has elevated the standards for end-of-life care in Washington and played a crucial role in reforming medical practice for the terminally ill since 1988. Their approach of providing direct service, advance planning resources, professional partnerships, community education and advocacy has helped transform patient-centered care.
